Current customers of our VPSI Sales Reps and physical locations can now place their orders online using our new digital order form. This order form is a PDF with editable boxes for you to type in your information and quantity numbers.

Instructions:

  1. Click the button below to open the form.
  2. Download the form.
  3. Fill out the first page with your customer information. Pricing is not included on the order form. If you would like a quote before we process your order, please indicate this on the first page.
  4. Add the quantity of each product you would like to purchase. Quantities are by the EACH, not the case.
  5. Save a copy of your form and email it to your VPSI Sales rep (find their email here) or PaigeS@vetpoultry.com.
